Injury Massage Therapy
At Elle, we are able to offer massage therapy billed to most major health insurance companies. Not all insurance plans cover massage therapy, so to find out if you are a candidate for this treatment, we can check your benefits for you. Simply bring in, fax, or call with your health insurance information.

Advanced therapeutic techniques, including deep tissue work, and hydrotherapy, applied to injured soft tissue, as prescribed by your primary care physician or chiropractor. Insurance Billing on Auto/ Workers-Comp/ L&I/ Most Private Health Insurance Plans Accepted: Aetna-Cigna-Corvel-Engage-First Choice-Group Health Options-LifeWise-Premera Blue Cross-Uniform as well as many out of network plans.

To verify your benefits, we will need the following:
- Your Insurance Card ID Number
- Your Date of Birth
- Your Full Legal Name
- The customer service number on the card
Once we’ve checked your benefits and confirm your individual plan covers massage therapy, we will need a referral or written prescription from your doctor or chiropractor (even though some plans tell you, the patient, that a referral is not necessary, we are required to send in one in order to receive payment).
Your Rx needs to include the following information:
- The referring Dr’s name and phone number
- The number of treatments being prescribed and frequency of treatments
- The diagnosis codes and areas of the body to be treated

Insurance Billing on Auto/ Workers-Comp/ L&I/ Most Private Health Insurance Plans is done by Elle’s staff. All co pays, deductibles, and co insurances are due at the time of service.
